Capture Alert Status for Each Vaccine

We need to identify the status of each alert when a vaccine is given. I.e. when a child is given Pentavalent 1, what is the alert status for Pentavalent 1?

This will allow us to easily report on whether vaccines are being administered on time (status of "due") or late (status of "overdue"), and whether all "due" vaccines are being given in the proper visit. This will allow us to report on the visit completeness rate; if 4 vaccines are due on a visit, whether all 4 are being administered or not.

I spoke with @keymane and @manutarus and we decided it was best to capture this in OpenSRP client and have that data transmitted upstream to the server and to the reporting layer.
