IPAL2014112100 for Moodle 2.8

This release of IPAL is for Moodle 2.8. It will not run with Moodle 2.7 or earlier versions. There are Apps for the Android and iPhone devices to run with IPAL. The code has been cleaned up after checking with the Moodle code checker and php code checker.

The add/change question interface is now cleaner, the code is cleaner, and the EJS App runs without an iframe with the size selected by the teacher.
