Updates readme documentation

Updates readme to reflect 0.3 updates:

• Removes references to cusom FileSystemProvider for Umbraco
• Adds sample configurations for FromConfigCloudImageService

Version Updates

• Removes custom Azure FS Provider taht pulled config from web.config (use the built-in support for that in the official package now please)
• Updates ImageProcessor library versions to latest, this resolves CloudImageService issues when using the library compiled against older ImageProcessor versions
