What are you changing in this pull request and why?

#4022, the first issue generated by this new template, presented some opportunities for refinement, predominantly:

  • markdown doesn't work in the template description, so I moved it to the markdown element
  • auto-assign me (@dataders)
  • make fields required
  • allow for submitters to provide the svg icon file (to save me time)
