Context

dbt-athena was forked in November 2022 from Tomme/dbt-athena/ in order to have a more community based approach - that doesn't rely on a single, but on a core group of maintainers that review/triage community contributions.

Since then the maintantners have done a great job keeping the adapter updated with the latest dbt releases - and helping the contributors to continuously ship new features and fix bugs.

#4022, the first issue generated by this new template, presented some
opportunities for refinement, predominantly:
- markdown doesn't work in the template description, so I moved it to
the `markdown` element
- auto-assign me (@dataders)
- make fields required
- allow for submitters to provide the svg icon file (to save me time)
