Generate extra metric json without TMA

Move save_form out of extract_tma_metrics to allow sharing. Move the
extra metric generation from the end of extract_tma_metrics and move
to its own function. Fix issues generating EMR, GNR, GRR and SRF perf
json with the latest mapfile.csv. Issues are addressed by adding
missing table lookup values.
