create README.rst, fix file saving

two minor changes:

• create README.rst (which was being loaded within setup.py but was missing, leading to failure)
• fix file naming for the files that are created

You can also still use pyneurovault, which has examples, readmes, etc. In case this was forgotten :)

also updated requirements to latest, as they were conflicting with newer code installs
