This is the adherence scale used by Adherence Counselors regularly. If a patient scores poorly here, they will get TRAT and likely Level of Care Assessment Tool

To load this form in CHT ;

• clone the project and navigate in the config folder of the project, also make sure a running CHT instance with cht-conf install
• Run cht --url=https://<username>:<password>@domain convert-app-forms upload-app-forms -- srq command to upload the form in CHT ie cht --url=https://medic:[email protected] convert-app-forms upload-app-forms -- srq
  Note:Be sure to replace the values and with the actual username and password of your test instance
