feat(hugr-py): AsCustomOp protocol for user-defined custom op types.

Replace existing custom operation types with this.

Follow ups:
- Similar thing for custom types.
- Optional: allow these types to register themselves with `serialization.ops.CustomOp` so they can be deserialized directly.
