Azurerm 1.x, Latest Script Extensions

• Upgrades the core azurerm resource provider to 1.0.1
• Removes lb_port var
• Upgrades the Linux and Windows compute extensions to the latest major versions

This adds new functionality to be able to provide a custom script extension. Also tested with azurerm provider 0.3.
