Right now in whatwg/html#2747 we're planning to hide nested levels beyond the second using CSS, but ideally we should just not output them.

Feedback from benschwarz/developers.whatwg.org#59 indicates maybe we should not do this. Instead we should in fact add a toggle for expanding the table of contents into a full table of contents. That would require no Wattsi changes.

So, I will migrate this issue to a new one on whatwg/html.
