[FEATURE] Show amount of mutes & warns when muting/warning someone

A lot of the time the people who get muted have a history of it which isn't clear unless the mod remembers, or checks their !notes.

Additionally mutes don't always get a warn, which can make people who have a history of violating the rules but only getting muted ^{(instead of muted & warned)} look less bad when a warn does happen.

Proposed solution: Keep a counter for both warns & mutes.

On mute:

[user] has been muted for [duration]. They have been muted [X] times & have [X] warns.

On warn:

[user] has been warned for: [reason]. They have been muted [X] times & have [X] warns.

Also, it's quite inconsistent atm. ​ Mute is an embed & doesn't show the mod's name, whereas warn is a normal message & shows the mod's name.
