It seems the situation as described in #11 has changed - etherpad-lite seems to store login information in a cookie named "express-sid".

To logout, this cookie must be deleted.

Can this be implemented in your plugin?

I'll accept good patches to add this in. I'm not really actively maintaining this plugin anymore as the need for it at work has gone away.
