Version 0.4

This release includes :

• the mkdocs documentation (https://docs.civicrm.org/civiproxy/en/latest/)
• fixes for issues #12 and #14
• has been tested with CiviCRM 4.7.22

It is still marked as a pre-release because we think we should have a proper audit first.
