Implement removal support

To be useful we actually need to be able to remove kernels too. The best strategy is for the library to expose simple methods to enumerate promoted $BOOT entries, and a method to delete them.

This will allow blsctl and moss to implement their own logic for handling removal (ie after a state prune)
