Add Treatment Notes and Patient Case to Appointments

Currently in the API I cannot find a way to link Treatment Notes and Patient Cases to an Appointment without first processing all Attendees and then linking internally.

This would be a good feature if they linked forwards and backwards to an appointment so we would be able to attach/update treatment notes per appointment with one process.
