Remove the possibility of duplicate app names in the Adaptor

As of now if you try to adapt the same XML twice then it results in a duplicate app name and a soft error. Considering that the name can only be changed in advanced mode, this should probably not be the case. Instead, it should probably try to auto-generate a name such as "Concept Mapper (2)", in a similar way to the translator.
