Add higher-order rules that can be parameterized by symbols, symbol strings and integers.

In general, higher-order rules are to generics what LALRPOP's macros are to syntactic macros.

Prolog's definite clause grammars can be easily parametric.
Menhir supports higher-order rules, for example

option(X):
  | { None }
  | x = X { Some x }.
