Debugging issue - MCH3601 unmonitored by QB5IDEAL

I'm currently reviewing Code for IBM i and IBM i Debug as our recommended IDE for our CL/RPG developers.
I'm having an issue though with the debugger. I have gone through the setup steps (certificates, start debug service, etc.), however when starting a debug session for a program, the debug session appears to come up (the icon bar with the step over/into/stop/etc. shows) but the program just gets executed without a chance to step into it (even when manually setting breakpoints). I can see the QB5SERVER job getting launched in QUSRWRK, but it shows an unmonitored MCH3601 error by Q5IDEAL. When this happens, and I stop the debug session and start a new one, on that new session the the debug icon bar does not even show anymore. When I end the QB5SERVER job, a subsequent debug session starts it again, I get to see the debug icon bar, but the behavior is again the same as above.

I am aware of APAR SE70567 but we do have the PTF that fixes this issue installed (we're on V7R3 - PTF SI76237)
Has anyone experienced this issue?

Environment:
OS V7R3
VSCode: v1.84.2
Code for IBM i: v2.4.0
IBM i Debug: v1.0.0

Turned out to be some unknown configuration issue. Full reinstall of VSCode etc. sorted this out.
