[Office Hours] Slider Questionnaire and Firebase Upload

Reproduction

Used Phoenix Builder to build a slider questionnaire, but when building on Xcode, questionnaire shows up without the slider.
Also trying to create a field where users can add their GCS score and have that upload to Firebase, but when I try to connect it, Previews crashes.
