Link hashtag using markdown #13

Right now, Mattermost link an hashtag (#) in the message to a channel. Let __HASHTAG__ be the actual hashtag in a tweet, the formatted message using markdown should look like:

[#__HASHTAG__](https://twitter.com/hashtag/__HASHTAG__)
