I occasionally get an “Upload Failed” message, like this one:

When it happens, there doesn’t appear to be a way to retry. Refreshing the page clears the form.

Also (and I’m sure I have previously seen a ticket for this) if I make a second attempt, I usually have to rename images in order to avoid this message:
