Make a generic NSIDC application db image

We have a few images like this one. There's nothing application-specific in this image, except for names, like the schema and database name. The needs are shared: a database, a schema, an application user, a support user, and pg_stat_statements. How can we genericize this image so we don't need to keep repeating this work?
