Add provider reference number to emails sent

What?

As a care provider, I want to make sure I don’t confuse two people with the same name / date of birth by knowing the reference number for the care provider.

Why?

Sometimes people will be more familiar with their own reference IDs than names and dates of birth

Very occasionally there are matches for first name, surname and date of birth.

It’s done when:

• GOV.UK Notify template created
• New information is put into the template when we send it out within the email care provider Lambda
