Hi Nathan,

I noticed in a comment on issue #2 that this example project is no longer supported; I presume since it is using a super old version of rails and hasn't been updated in 7 years.

Unfortunately, the current README for cocoon links to this repo and implies this is a current working example of how to use cocoon with Formtastic.

I have created Pull Request #4 to address the "unsupported" application, by upgrading the demo to Rails 5.
