.cod file not being created

What steps will reproduce the problem?
-Follow through the given tutorial at the end of your thesis with Arc 10

What is the expected output? What do you see instead?
-At part A.8 of the appendix, the .cod file is not created (the .dbf are 
created without issue in the previous steps), and therefore it is impossible to 
continue

What version of the product are you using? On what operating system?
-latest version on Windows 7 - 64 bit w/arc 10

Please provide any additional information below.

-This toolbox looks REALLY interesting, and I'd really like to use it to its 
full potential, but no matter what I try the .cod file isn't created.  I 
thought it might be a problem with arc not recognizing the file type, but the 
file is simply not created, so I think it has something to do with the toolbox. 
 Any help would be greatly appreciated.
