Popup UI

Currently the popup UI displays information inconsistently. It also doesn't show url's that have already been acted on (but a badge count still appears).

I propose to display all third party leaky requests and the option that was selected every time the extension badge is clicked (similar to how ghostry does it). A user can then change their mind about a request, and select a different option, etc.
