I have problem with:
[url=xxxx_xxxx.com]site name[/url]
url's without underscrote converted well.

We're using the built-in filter_var, as seen here: https://github.com/milesj/decoda/blob/master/src/Decoda/Filter/UrlFilter.php#L79

This looks like a bug in PHP itself: https://bugs.php.net/bug.php?id=64948
