I can confirm this to still be the case for the mgba core.
When creating an override that automatically enables Runahead for this core, if Second Instance Mode is not enabled as well, any save you make or update will not be retained, essentially resetting all progress made in the meantime.
When run-ahead is enabled and second instance is disabled, in-game SRAM is not updated either upon saving or closing the core and Retroarch.
Issue only affects GBA games, GB and GBC are fine.
Tested on commit 4fdfa33, though older builds have the same problem.
Issue was likely masked due to the fact that 4fdfa33 fixed audio related issues that also prevented using run-ahead with a single instance reliably.
All tested is NTSC.
Retroarch stable 1.9.11, as well as latest nightly as of now, and stable 1.8.9.
