No exists 'multicheckbox' and modules folder

Hello, installing this application i had have two errors:
1º - Modules folder doesnt exists.
Create forlder under "application/" solves the problem.
2º - File 'application/forms/example.php'.
Rename to Example.php
3º - 'multicheckbox' error doesnt exists.
Change to 'multiCheckbox', solves the problem.
