Dedupe handling and subscription cancellation bugfixes

This release fixes two bugs:

1. Deduplication was not being handled, leading to failing recurring transactions if the id of the contact with the recurring contribution was changed.
2. Subscription cancellation (cancelling a recurring contribution series) was not being handled, leading to inaccurate messaging.

Bugfix release

The release contains two small but import bug fixes:

1. ACH/EFT works correctly on event pages
2. A bug for recurring contributions with a non-empty number of installments (issue #19)

It also updates the ACH/EFT graphics and now correctly shows the status of in-progress recurring contributions.

This release has a number of small bug fixes and enhancements, primarily related to ACH/EFT code, listed here:

1. ACH/EFT now works with events and non-recurring contributions.
2. ACH/EFT verification is fixed.
3. The method of addition of the navigationMenu item has been fixed.
4. Multisite domain code fix.
5. Eliminated the incorrect warning when changing a recurring contribution amount of an existing recurring contribution.
6. The ACH/EFT billing form fragment currency/country is now configurable, with improved versions of the USD and CAD ACH/EFT billing form.
7. Improved iATS Admin page.

Note that ACH/EFT is not supported yet for non-USD/CAD currencies.
