Este es el formato: https://help.github.com/articles/creating-issue-templates-for-your-repository/

Así como @federivo hizo el de PRs, deberíamos tener el de Issues también.

Una vez definido, se necesitará agregar el template a este repositorio y luego replicarlo en cada uno de los repositorios.
