This is related to a change in the proximity sensor range, from 5cm to 10cm. As development for cm-13.0 is stopped, please consider either going one version back (20161122) or upgrading to the cm14.1 rom.

In the way you are describing your problem, I can assume you might have a faulty proximity sensor. Does that happen on stock ROM too ?
