Create an "SSO Onboarding" form as part of SSO registration

Want to keep the registration process more "inline" on the dashboard and have a contact us form pop up within the dashboard itself.

Need to decide if we'll make a stand alone form that tries to get the required details right away
Need to review the impact on compliance. Should be fine here but there will be a new third party integration.
Maybe we just make the Contact Us page support being in an iFrame? Not really sure.
