Review thread safety of the FATX driver

While most of the FATX driver functions acquire a lock before doing any work, there are some Nt/Io functions that also access some file-related variables (in particular, those stored in the file object). Because of this, the code is probably not completely thread-safe and needs to be reviewed in that sense.
