Positioning plugin: Disable icon by default in courses

When enabling the positioning plugin, the new tool that appears in all courses is directly visible to students. This is confusing to most if you have a portal with hundreds of courses and teachers.

Make sure when the plugin is installed for the first time, the course tool is hidden by default, so course admins decide whether to enable it or not (which requires several changes to work).
