Support scratchpad.delete, scratchpad.create from SMART Web Messaging

After #91, add support for operations that add/remove items from the draft orders scratchpad. When a scratchpad.delete or scratchpad.create message arrives from an app launched from the "Radiology Orders" tab, the in-progress draft orders should be updated appropriately.
