It seems like Twitch has thwarted Brave with a recent update to their ad server.
I did find one tool that swaps the 1080p stream for a 480p stream until the ad is done playing: https://github.com/saucettv/VideoAdBlockForTwitch
Could we implement something like that in the Brave core?

Currently not, we'll implement via js snippets (like uBO) the previous fix worked temporarily. But twitch is quick to close loopholes.
The above method still shows the advert, just in lower resolution? Not much of a workaround when the ad still shows tbh
Sorry, something went wrong.
For some reason they don't show ads if you're at 480p resolution, but really enforce ads at 720p-1080p.
