So I've seen an issue below that reported the same thing: regular interval stuttering that makes the game unplayable.
Clues:
I am using Windows 7 SP1 x64
Using Diablo 2 - LoD v1.14d with PlugY v14.02
The stutters calm down after a while - very interesting. If you leave the game running for 10 minutes and return to it, the stutters become so small that they are barely noticeable (though still there)
Sometimes when the stutters happen, black spots appear on the bottom of the map (not the UI) or in other directions (depends on where you're running with your character)
Happens both in windowed mode and fullscreen.
Happens both with VSync on and off
No option changed in the CFG fixes it
It would REALLY help a lot of people if you fixed this, because this is BY FAR the best wrapper out there. It is so smooth when it's running correctly - the FPS is godly. Not to mention that it works quite well with perspective on. Massive congratulations for your work.
If you need any further info, please tell me - It would be godly to have this fixed and to rock the game so smoothly in a widescreen setup that actually doesn't break the AI. You've done the impossible already, so THANK YOU!

Having same issue on Windows 7 Pro 64 bit. My current configuration uses basemod, BH Maphack, and the uber tristram minion fix. I have the FPS fix disabled in D2DX and Basemod as BH Maphack seems to have the best quality fps fix of the three. Having D2DX or Basemod's FPS fixes on results in tons more stuttering. Have tried updating drivers, messing with all config settings, and even trying a clean boot in windows, nothing works.
However, going to the in game video options and changing lightning quality from high to low decreases the frequency of the stutter dramatically. In my case it changed it from once every 3 seconds to once every 20 seconds. Hopefully that helps to track this down.
