Rule failure telemetry documentation

Need:

Support team on a day-to-day basis checks the rule failure telemetry to proactively identify issues before they being get reported from clients. It has been raised by them several times that rule failure telemetry page is not easily understandable.

AC:

• Esp., add details of what each column in the Rule failures tab in App designer means, esp., the columns Individual UUID, Rule UUID, Source, Entity
• Cases when any of the above columns could be empty and what it means
• Also add any other relevant details like how rule failures in DEA can be checked, etc.,
• Add it under the How do I section available for implementors

• Source type and Rule type seem to semantically duplicate.
• App Type should be mandatory in the database. Web app is not populating it currently.
• There should be constraint for null allowed only for ReportCard type errors.
• remove org.avni.server.domain.enums.ruleFailure.SourceType#ReportCard as this is not used or start using it correctly
• Not sure all entity type and source types are used
