Having unique constraint on multiple fields

Motivation

Poshan sathi users who share the same locations, often trade their location to visit beneficiaries on the field. They then end up registering the individual on the device again, creating a duplicate.
The issue is only identified when the supervisors filter out the report for the month and they see there are beneficiaries with the same name and DOB.
The users should not be allowed to register duplicate individuals.

Acceptance Criteria

The current solution provides only having a unique "Name" which is not applicable in this case, as there are multiple beneficiaries with the same name.

Having a unique constraint on name and on DOB or on an other field should solve the issue.
