Change Shell header

Current Shell headers:

==> default: ---------------------------------------------------------------
==> default: CakePHP v3.0.0-RC2 Console
==> default: ---------------------------------------------------------------

User feedback indicates it's annoying and that provisioning readability could be improved by changing it to something like this:

==> default: ---------------------------------------------------------------
==> default: Creating application http://cake3.app
==> default: ---------------------------------------------------------------

==> default: ---------------------------------------------------------------
==> default: Installing additional apt package 'dos2unix'
==> default: ---------------------------------------------------------------
