Iteration form inconsistencies #138

Here are a bunch of UI problems with iteration creation, feel free to split into dedicated issues if you want:

  1. The Edit form says "Iteration", while the Create form says "Iteration Number", and the All Iterations table says "Name"
  2. The order, placement, spacing, and size of fields are completely different between the edit and create forms - this is really ugly
  3. Date inputs that are disabled have a white background. The disabled estimate input has a gray background.
  4. The Create Requirement form has a "Save" button, but the Create Iteration form has a "Submit" button, and creating a release number has a "Create" button
  5. The purpose of the table in the Edit Iteration form is not clear - I actually totally blanked on what it was supposed to be. Maybe a title above the table would help ("Scheduled Requirements" or something).

Each label now says "Iteration Number"
