installed it as per instructions, no errors generated, but plugin doesn't work

Plugin worked great for me in Roundcube 1.6.6 then I upgraded to Roundcube 1.6.8 and can't get it to work anymore. I've re-installed it with composer, using twofactor_gauthenticator version "dev-master", and in Roundcube in settings-> about it shows that it's loaded, but there is no option to configure it (enable).
