Support dynamic reference data #94

Presently, reference data is held statically within code. This work would push reference data down into the database to make it dynamic.

Current list of reference data used by the service:

DataSource
FinancialType
FundingProgramme
Milestone
ObservationType
OutputType
OutputMeasure
OutputTypeMeasure
SchemeType
