Bug Report 🐛

Context

We have migrated to Discord from Slack (see accordproject/technical-steering-committee#18)

Detailed Description

References to Slack should be updated to reference Discord, with the link of https://discord.gg/Zm99SKhhtA

We can also remove the button at the top of the README for https://accord-project-slack-signup.herokuapp.com

We can also update the repository's wiki (https://github.com/accordproject/{REPOSITORY}/wiki)
