I was having trouble trying to add Sirius's models, and encountered unexpected crashes. I looked in the filelist.txt and noticed that nearly half of the files present in the extracted folder were not present in the filelist. Alongside this, numerous entries had the same index number.
To replicate, extract and rebuild dvdbnd1. Dark Souls will crash on startup.
