add a field to optionally capture the github user name of the submitter? #6

When looking through issues in the Feedback repo, its often hard to tell who submitted the feedback (which is good long term for non-internal folks that want to give Translator feedback, probably :) ) -- at the moment, as we do internal testing, it would be really handy to capture the person who submits the feedback so we can follow up with them if we need clarification of their feedback.

For example: NCATSTranslator/Feedback#36 -- I can rerun the query, and see evidence for "Estrogens, conjugated synthetic a" right away with links to PubMed. So I want to clarify with the submitter if 1) the chemical needs to be "estrogens" (as in, I want to see results by "chemical role"), or 2) if the system wasn't working at the time and no results were shown or 3) if the evidence wasn't available for the record returned, etc...

An alternative might be to request more meta data or a template for filling out the feedback (too heavyweight?).
