As per this changelog, we can now attach reasons to certain actions that will appear in the audit log.
This should be added.
Be that as it may, I sure as hell can't find any documentation for it so far. Will continue searching.
discord/discord-api-docs#265 shows that it might be using the X-Audit-Log-Reason header?
