WP Forms / Recurring Donations not Recurring

I used WP Forms in conjunction with Stripe to set up out new donation form on our website. I RTFM and followed instructions to include the recurring donation option.

Expected: Someone says "I want to donation $100 and that should be a recurring donation every month"
Actual: It seems like we get the first donation but it never recurs.

@norcross If you get a moment can you just look over my form set up and see if I did something wrong with the logic? Again, I think I did what it told me to do, but maybe I did something wrong.

question: has anyone said they did recurring and it didn't run that way? as a test or otherwise.

Yes, and we have receipts which I can show you where they selected Recurring as an option they selected but then nothing.
